Tennessee OT Licensing and the Compact

Tennessee began issuing OT Compact privileges on May 20, 2026, with a state fee of $125 for OTs and $100 for OTAs. For a full license, Tennessee's reciprocity route needs verifications from every state you have held a license in, a school transcript and a vendor background check, and files are reviewed at board meetings. Both routes, plus Tennessee's modality certification rules.

Tennessee began issuing OT Compact privileges on May 20, 2026, with a state fee of $125 for OTs and $100 for OTAs plus the $75 Commission fee. A Tennessee license by reciprocity requires your transcript, NBCOT results, verification from every state where you have held a license and a vendor background check. Files are reviewed at board meetings.

Tennessee has two routes for an occupational therapist or OTA licensed elsewhere. If your home state issues OT Compact privileges, you can obtain a Tennessee privilege; Tennessee began issuing them on May 20, 2026, according to otcompact.gov. Otherwise, you apply for a Tennessee license by reciprocity through the Tennessee Board of Occupational Therapy. Reciprocity requires your school transcript, NBCOT results sent directly by NBCOT, verification from every state where you have ever held a license, and a criminal background check from the board's vendor.

How does a Tennessee compact privilege work?

The Commission's rules apply: you need a home state where you primarily live, that is a member and issues privileges; an unencumbered home-state license; and an FBI background check through your home-state board. You apply in CompactConnect yourself and pay the $75 Commission fee plus Tennessee's state fee, which the Commission lists at $125 for an OT and $100 for an OTA. Third parties cannot complete the application for you; the Commission says licensees must create their own accounts, complete the application and pay themselves.

The privilege expires when your home-state license does, and you must purchase a new one each time you renew that home license. See the OT Compact hub and renewing OT Compact privileges.

What does Tennessee reciprocity require?

Tennessee Rule 1150-02-.05(2) sets out the reciprocity procedure. The applicant:

  1. submits the application with the non-refundable application fee and the state regulatory fee;
  2. includes a passport-style photograph taken within the preceding 12 months;
  3. has the degree-granting school send a graduate transcript directly to the board, showing the degree conferred and carrying the school's seal (the program must have been accredited at the time the degree was granted);
  4. has verification of licensure status sent directly to the board "from all states in which the applicant is or has ever been licensed";
  5. has NBCOT send examination results directly to the board;
  6. discloses any criminal convictions (other than minor traffic violations), licensure denials or discipline, loss or restriction of a license, and civil judgments or settlements as a defendant, including malpractice; and
  7. has the background check vendor named in the application materials send the result directly to the board.

The rule also says personal resumes "are not acceptable and will not be reviewed," and documents in another language need certified translations. Once approved, you must pay the initial licensure fee within 30 days of being told it is due, or the file is closed.

Every license, ever

The verification requirement is the one that stalls Tennessee files. A license from a short contract years ago, now expired, still needs a verification. Some states provide primary-source verification online; others require a request and a fee. See license verifications between OT boards.

Board meeting timing

The rule says completed files "will ordinarily be processed at the next Board meeting scheduled for the purpose of reviewing files." A file that is complete a few days after a meeting waits for the next one. No one can promise which meeting a file will make; the practical move is to have everything in early.

What does it cost?

The fee schedule in Rule 1150-02-.06, as published, lists an application fee of $100 for an OT and $75 for an OTA, biennial renewal of $85 and $60, a $10 state regulatory fee each biennium, and a $25 limited permit fee. Confirm current amounts with the board before paying.

Does Tennessee require modality certification?

Yes. Tennessee requires separate certification to use electrical stimulation and thermal agents. Under Rule 1150-02-.04(4), you qualify with current Hand Therapy Certification Commission certification, or with board-approved training: 25 contact hours including five supervised patient treatments for electrical stimulation, and 20 contact hours including ten supervised treatments (five using ultrasound) for thermal agents. Training must be completed within the two years before you apply for the certification. If modalities are part of your job, request the certification with your license application. See physical agent modality rules by state.

What if it has been a while since you passed NBCOT?

If you apply more than three years after passing the NBCOT exam and are not currently practicing in another state, the board may impose added requirements, including a self-assessment, ten hours of formal learning for each year since you qualified, or continuing education in ethics, Tennessee jurisprudence and documentation. Once licensed, such a licensee must be supervised by a licensed OT in good standing for the first twelve months of practice (Rule 1150-02-.04(3)). This does not apply if you are actively practicing in another state and applying by reciprocity.

How do renewals work?

Tennessee uses a birthdate system: your license renews by the last day of your birth month, every two years (Rule 1150-02-.09). You attest to 24 continued competence credits earned over the two calendar years before the renewal year, and up to four excess credits may carry over to the next period (Rule 1150-02-.12). If you also hold a compact privilege elsewhere based on a Tennessee home license, that privilege follows your Tennessee renewal date.

Privilege or license in Tennessee?

A privilege fits if you live in another issuing state, intend to stay, and want to treat Tennessee patients in person or by telehealth. A license fits if you are moving to Tennessee, your home state does not issue privileges, or you need Tennessee's modality certification, which the rule ties to meeting Tennessee's licensure qualifications and application procedures. See endorsement or the compact.
